You don't have enough options. I don't mostly use it to mean one or the other; I intentionally use the word's essential ambiguity to mean "angry to the point of being insane or possibly insane in a loud, hand-waving, mouth-frothing manner".

If I meant "angry" or "crazy" that's what I'd say. 'S plenty of words in the English language(s), might as well give them all a workout.

I should expand, only my friends are mad (insane). Someone genuinely insane has a whole bunch of specific terms, usually pertaining to their situation (schizofrenic, MDS, bipolar, catatonia, etc.).

But then, I come from a country that has been known to use 'bastard' as a term of friendliness.
viz. "Here's the old bastard now, how ya goin' mate?", so it could be said my perspective on English is skewed by my native culture. :)
